Resettlement PROGRAMME
Accommodation and Support
Ty Gobaith currently has sixty six units of accommodation. The rooms are used for a variety of purposes depending on the needs of the services users.
Self Catering Accommodation
Ty Gobaith has recognised the need to promote the independence of the service users who live here. In 2005 we underwent an
extensive refurbishment which resulted in thirty seven of our rooms becoming self catering. This allows service users to cook
and clean for themselves which encourages the learning of life skills which have either been eroded over time or not learnt
in the first instance.
Bridge programme Accommodation
The Bridge programme is a 3 phase bio-psycho-social substance misuse programme and access to these rooms is dependent on the
outcome of a different assessment and referral process. please see the relevant website area for further details.
Half Board Accommodation
The majority of our half board accommodation is set aside for Cardiff County Council Temporary Accommodation department.
Resettlement Units
There are six self contained units for the Resettlement programme, one of which is set up to enable a Service User to have a dog with them. These rooms are nominated for Service Users by the Resettlement Team as they are ideal for further training in life skills for independent living and also for any specific support need such as dietary, health, mobility, religious needs etc.
When a Service User is referred into the centre a comprehensive assessment is carried out to ensure suitability for the centre this also allows for the beginning of a structured tailored support plan. A full induction and tour of the centre is completed highlighting house rules, charter of rights, service user participation, service user expectations, all the different programmes the centre has to offer, centre activities, etc.
New Service Users to the centre are allocated a Resettlement Worker to meet with on a weekly basis in order for needs assessments to be carried out. promoting Independence and Empowerment is the main focus of the Resettlement Workers work with Service Users. Individual action plans and goal setting are completed between the Service User and Resettlement Worker. This ensures that the Service User has an active role in their support and ensures that the long and short term aims are achievable for the Individual. Measuring these action plans is equally important to ensure that the support need of the Service User is being met and to see what progress is being made. T? Gobaith strives to be a place of change for residents and all resettlement workers receive training in motivational interviewing as well as Cognitive Behavioural Therapy techniques. This aims to help empower service users with the confidence to engage in change. If access to training, employment or substance misuse services is required these can be facilitated in house via either The Get Skilled programme or the Bridge programme.
The resettlement team is able to provide advice and support around the following issues.
Once a week Resettlement Meetings take place to discuss Service User progress. This ensures that the support offered is a team effort in Promoting Service User independence. This benefits both the staff and Service Users reach their goals.
Ty Gobaith is currently assessing the way we support our service users and the introduction of an outcome assessment tool is planned for the near future. It is hoped that this will further enable the change process for our service users.
